Consultation for transferring to another hospital (forward coordination)
Our Community Medical Liaison Office serves as a window for forward coordination with you. We provide transfer consultation for those who need sub-acute or chronic care after completing acute treatment, rehabilitation, or terminal care.
《Our hospitalization system》
1. The Community Medical Liaison Office accepts referrals of patients and consultations on transfers over the phone.
2. Discussions will be held based on the hospital's acceptance indications, available beds, and other factors.
3. We will get back to you as soon as possible with information on whether or not we can accept your application and when we will accept it.
※This does not apply to emergency hospitalization.
Discharge support (backward coordination)
The Community Medical Liaison Office has a discharge support section where medical social workers and discharge support nurses provide discharge support to hospitalized patients and their families. Each patient is assigned to them and is supported through various consultations from the time of admission to discharge from the hospital. We also work closely with care managers in charge of long-term care insurance, long-term care insurance service offices, long-term care facility staff, and other external organizations to provide discharge support.
Dear Patients and Families
Medical social workers (certified social workers), who are specialists in consultation support, provide various consultations related to medical care, welfare, and nursing care for patients and their families who are visiting or staying at our hospital. In addition, medical social workers and discharge support nurses provide support to patients and their families so that they can live a comfortable life after discharge from the hospital.
